Requesting a copy of your records from Green Cardigan Counseling:
If you are a current or past client and need a copy of your records from Green Cardigan Counseling, please contact me via email at greencardigancounseling@gmail.com
I value the confidentiality of the work conducted in-office and deeply respect your right to privacy. The process of obtaining your records will require a one-on-one meeting during which we may discuss the nature of the information you are seeking and formalize a plan to provide that information to you in a way that protects your privacy as much as possible. Records will only be provided in-person to the former or current client requesting them and in a sealed envelope. The current or former client requesting records and the provider will be required to sign two (2) copies of a receipt to indicate that the records were provided and received, at what address, and on what date. The current or former client requesting records will keep one receipt and the provider will keep the other.
Any extenuating circumstances will be addressed on a case-by-case basis.
